Fluconazole-resistant cryptococcal meningitis.

چکیده
A 40-year-old HIV-positive man was admitted to Cecilia Makiwane Hospital in December 2001 with a history of severe headache, photophobia and fever. Indian ink staining of cerebrospinal fluid (CSF) revealed yeast cells with 3 5 budding forms per high-power field. A diagnosis of cryptococcal meningitis was made and the patient was started on amphotericin B. He reacted to this almost immediately, and was observed to have a spiking temperature, tachycardia, and dizziness. He was changed to oral fluconazole 400 mg daily. He improved and was continued on 400 mg of fluconazole for 2 months, and then 200 mg daily.

Correlation of anti-fungal susceptibility with clinical outcomes in patients with cryptococcal meningitis
BACKGROUND This study aimed to investigate the correlation of minimum inhibiting concentrations (MICs), obtained by broth micro-dilution, and clinical response in patients with cryptococcal meningitis. METHODS Using retrospective analyses covering the period 2001-2010, factors affecting clinical therapeutic cure in patients with cryptococcal meningitis 10 weeks after the start of anti-fungal ...
متن کاملEmerging fluconazole resistance: Implications for the management of cryptococcal meningitis
We present the case of an HIV-seropositive individual with cryptococcal meningitis who was found to have a fluconazole resistant strain of Cryptococcus neoformans. The individual required multiple rounds of amphotericin and fluconazole 800-1200 mg after several episodes of clinical relapse. Cerebrospinal fluid sterilization was achieved and maintained with high doses of fluconazole. This case d...
متن کاملCombination therapy with fluconazole and flucytosine in the murine model of cryptococcal meningitis.
This study elucidates the role of combined fluconazole and flucytosine as therapy for cryptococcosis in the murine model of meningitis. Three strains of Cryptococcus neoformans for which the range of fluconazole MICs was wide--2 microg/ml (susceptible strain), 8 microg/ml (moderately susceptible strain), and 32 microg/ml (resistant strain)--were used for infection. One day postinfection, the mi...
متن کاملSymptomatic relapse of HIV-associated cryptococcal meningitis in South Africa: the role of inadequate secondary prophylaxis.
OBJECTIVES Cryptococcal meningitis is the commonest cause of adult meningitis in Southern Africa. A sizeable proportion of this disease burden is thought to be due to symptomatic relapse of previously treated infection. We carried out a study to examine the contribution of inadequate secondary fluconazole prophylaxis to symptomatic relapses of cryptococcal meningitis. DESIGN A prospective obs...
متن کاملChallenges in diagnosis and management of Cryptococcal immune reconstitution inflammatory syndrome (IRIS) in resource limited settings.
In many resource-limited settings, cryptococcal meningitis (CM) contributes up to 20% of all deaths with further complications due to Immune Reconstitution Inflammatory Syndrome (IRIS). We present a case report on a patient who developed CM-IRIS and then subsequent CM-relapse with a fluconazole-resistant organism and then later CM-IRIS once again, manifesting as cystic cryptococcomas, hydroceph...
